Visham jvara (Recurrent or irregular fever) in Sushruta Samhita

Visham jvara (fever) is described in the Samhita in Utt. 39, 30, 32, 110-128.
Such recurrent or repeated fits of jvara (fever) are caused due to the presence of toxic or morbid substances in the body system, due to intake of wrong articles in the diet or due to psychological causes like strong stress, grief etc.
Six types of visham jvara have been described in the Samhita. The symptoms and periodicity of recurrence of jvara (fever) differs in these six types. In all types, the jvara (fever) runs its appointed time course until the causing condition is consumed by the jvara (fever) and eliminated from the body system. Special febrifuge mixtures, tonics etc. (Utt. 39, 110-120) are also prescribed to give immediate relief as well as long term benefits
All treatments for general jvara described in the Samhita are applied for relief from pain and troublesome conditions in the body. These also conserve vitality of the body until body system cures itself. All 6 types of visham jvara are presribed same methods without any specific treatments SANTATA JVARA (REMITTANT FEVER=PARATYPHOID?) (Utt. 39, 30)-
This is continuous jvara (fever) with a high skin temperature and daily periodicity. It lasts for 7, 10 or 12 days with two distint aggravations in the course of day and night. ANYEDUH JVARA (Utt. 39, 31)-
This type is characterised by jvara (fever) every day followed by remission. TRITIYAKA JVARA (Utt. 39, 25)-
This type is characterised by jvara (fever) every third day. CHATURTHAKA JVARA (Utt. 39, 25)-
This type is characterised by jvara (fever) every FOURTH day. PRALEPAKA JVARA (Utt. 39, 22)-
This type is characterised by slow jvara (fever) every day in which there is a gradual loss of bodily elements producing emaciation, weakness, loss of vitality and ultimately death as in case of shosha (consumption). VIPARYAYA JVARA (Utt. 39, 23)-
This type is caused due to deranged bodily doshas being simultaneously located in two or four seats of the deranged bodily kapha.
It is characterised by sudden and irregular attacks of jvara (fever) with high temperature without any specially weakening effect on the system. This type is generally very hard to cure.

Dvandvaja jvara (fever) in Sushruta Samhita

Dvandvaja jvara (fever) is described in the Samhita in Utt. 39, 19-21.
This type of jvara (fever) is caused due to simultaneous derangement of two doshas. Symptoms are specific accordingly for the three sub-types described in addition to general symptoms of jvara given in the Samhita.
In addition to general treatment described for jvara in the Samhita, measures to balance the deranged doshas alongwith use of tepid water for drinking (avoiding the cold water) is prescribed. VATA-PITTA JVARA (Utt. 39, 19, 101-102)-
This type of jvara is caused by simultaneous derangement of vata and pitta doshas.
It is characterised by frequent yawning, abdominal distension, fits of unconsciousness, shivering, pain in joints, gradual emaciation, delirium and high skin temperature.
Specific treatment measures for balancing vata and pitta doshas alongwith special febrifuge mixtures given in Utt. 39, 101-102 are prescribed. VATA-KAPH (SHLESHMA) JVARA (Utt. 39, 20, 95-96)-
This is caused due to simultaneous derangement of vata and kaph doshas.
It is characterised by bodily pain, cough, expectoration, coryza, shivering, sensation of cold, lassitude, aversion to food.
Specific treatment measures for balancing vata and kapha doshas alongwith special febrifuge mixtures given in Utt. 39, 95-96 are prescribed. SHLESHMA (KAPHA)-PITTA JVARA (Utta. 39, 21, 97-100)-
This is caused due to simultaneous derangement of kaph (shleshma) and pitta doshas.
It is characterised by copious perspiration followed by cold, cough, vertigo, lassitude and nausea.
Treatment described for dvandvaja jvara and specific treatment measures for balancing pitta and kapha (shleshma) doshas alongwith special febrifuge mixtures are prescribed.

Tridoshaja jvara (acute fever) in Sushruta Samhita

Tridoshaja jvara (fever) is described in the Samhita in Utt. 39, 14, 16, 103-109.
Such fever is caused by simultaneous derrangement/aggravation of all three dosha (vata, pitta & kaph). Some experts suspect such jvara to be typhoid fever.
Apart from usual symptoms of jvara (fever), this type is characterised by insomnia, vertigo, difficulty in breathing, drowsiness, torpor, aversion to food, increased thirst, delirium, numbness, burning sensation & shivering, cardiac pain, partial insanity, blackish coating on teeth & tongue, pain in head & joints, dialation of pupil, clouded eyes, ringing inside the ears, inflammation of nasal & oral channels.
This type of jvara (fever) is extremely difficult to cure. In addition to general treatment practices for jvara described in Samhita, use of emetics, purgatives and intake of filtered clarified butter alongwith special febrifuge mixtures described in Utt. 39, 103-109 are prescribed.
Four sub-types of tridoshaja jvara are described in the Samhita though all these are treated by general method given for Tridoshaja jvara. ABHINYASA JVARA (Utt. 39, 14-15)-
It is additionally characterised by extreme torpor and drowsiness with low or normal temperature. HATAUJASA JVARA (Utt. 39, 15-16)-
It is additionally characterised by very loww or normal skin temperature and greatly reduced vitality. SANNYASA JVARA (Utt. 39, 16)-
It is additionally characterised by low or normal temperature and a marked inertness of muscular movement. OJONIRUDDHA JVARA (Utt. 39, 17)-
It is additionally characterised by marked shivering, numbness of sensation, fits of unconsciousness and delirium. Such jvara takes a turn for better or worse on the 12th day from the onset.

Agantuka jvara (fever); Gambhira jvara (fever) in Sushruta Samhita

AGANTUKA JVARA
This is described in the Samhita in Utt. 39, 34-36, 73-74.
In such jvara, dosa(s) derranged depends on some primary cause.
This type of jvara is caused due to injuries, poisons, smells (e.g. Hay fever), strong passions and as secondary effect of other diseases.
The characteristic symptoms of this type of jvara are darkening of the face, burning sensations, cardiac pain, diarrhoea, aversion to food, unusual thirst, pain in joints, extreme weakness, sneezing, drowsiness, delirium and gradual emaciation.
Such jvara is treated by methods described for treatment of jvara in general in Sushrut Samhita (Utt. 39, 47, 67). In addition, specific measures to counteract the specific cause e. g. wound, infection, poison etc. are also applied. GAMBHIRA JVARA
This type is described in the Samhita in Utt. 39, 37-40.
In such jvara, all the three dosas are derranged.
in addition to usual symptoms of jvara (fever), this type is characterised by extreme burning sensation, excessive thirst, constipation, laboured breathing, cough, paleness of complextion, dulness of sense perception and mental depression.
Methods described for treating jvara in general (Sushrut Samhita Utt. 39, 37-40) alongwith those described for treatment of Tridoshaja jvara (Sushrut Samhita Utt. 39, 103-109) are used in case of Gambhira jvara.

Kaphaja jvara (fever) in Sushruta Samhita

Kaphaja jvara (fever) is described in the Samhita in Utt. 39, 13, 90-96. This type of jvara is caused due to aggravation of kapha dosa.
In addition to general symptoms of jvara (fever), this type is characterised by heavy feeling in limbs, shivering, nausea, glossiness of eyes, gooseflesh, excessive sleep, sweet taste in mouth, low temperature and indigestion.
In addition to general treatments for jvara (fever), kaphaja jvara is treated by drugs given for mild emesis alongwith use of special febrifuge mixtures described in Samhita (Utt. 39, 90-94).

Pittaja jvara (fever) in Sushruta Samhita

This type of jvara (fever) is described in the Samhita in Utt. 39, 12, 82-89.
Pittaja jvara is caused due to aggravation of pitta dosha.

This type is characterised by high temperature, vomitting, diorrhoea, oral inflammation, perspiration, disturbed sleep, delirium, fainting, burning sensation, pungent taste in mouth, slight jaundice and vertigo.
Special treatment for this type requires use of purgatives and use of special febrifuge, pastes & gargles given in Sushrut Samhita (Utt. 39, 88-89).

Vataja jvara (fever) in Sushruta Samhita

It is described in the Samhita in Utt. 39, 11, 40, 75-81.
Its cause is derangement of Vayu dosha.
In addition to general symptoms of jvara (fever), this type is characterized by shivering, irregular rise of temperature, dryness of the mouth, insomnia, hot & dry skin, pain in the head, chest & limbs, bad taste in mouth, constipation, colic pain.
In addition to general treatment of jvara (fever), this type is treated by portions of filtered clarified butter that have been matured for some time to be given as soon as first symptoms appear. Special ferbifuge mixtures and external medicines for the treatment of this type of jvara are givin in Sushrut Samhita (Utt. 39, 75-81).

Fever (Jvara) in Sushruta Samhita

Classical Ayurvede text Sushruta Samhita describes fever (jvara) in Utt. 39.

Its onset is marked by a sense of fatigue, aversion to physical effort & food, pale complexion, goose flesh, bad taste in mouth, frequent yawning, aching limbs, lachrymation, feeling of heaviness, sense of depression and sensation of cold.

When fever (Jvara) becomes established, normal perspirration stops, skin temperature rises and breath becomes hot.

General principles of treatment of fever prescribed are light and restricted diet; internal cleaning of the system; special regimen of rest and conduct; use of antiseptic, febrifuge & tonic medicines; use of milk, cereals & meat in diet (Utt. 39, 47, 67).

General prohibitions when fever is established include stoppage of bathing or sprinking water on the body; no use of cosmetics & creams, oily foods, sleep during daytime, cold drinks; stoppage of physical excercise & sexual intercourse; no strong emetics and purgatives.

Samhita describes 8 types of fevers (jvara): Vataja, Pittaja, Kaphaja, Tridoshaja, Dvwndaja, Vishama, Aagantuka, Gambhira.

In addition to general symptoms, treatment principles and prohibitons, there are specific diagnostic features, cause and treatment principles for each of the 8 categories of fever.
